Breast Cancer Radiation Therapy

Radiation therapy uses carefully planned high-energy radiation to destroy cancer cells and reduce the risk of breast cancer returning. Treatment is personalized according to the type and stage of cancer, surgery, pathology and lymph-node involvement.

1. What Is Radiation Therapy for Breast Cancer?

Radiation therapy is commonly given after breast surgery to reduce the risk of cancer returning in the breast or nearby lymph nodes. It is generally painless and you are not radioactive after external-beam treatment.

2. When Is Radiation Therapy Recommended?

Radiation may be recommended:

  • After breast-conserving surgery (lumpectomy)
  • After mastectomy in selected patients
  • When lymph nodes are involved
  • When there is a higher risk of recurrence
  • For locally advanced breast cancer

The decision is individualized based on pathology, surgery and other clinical factors.

3. Radiation After Breast-Conserving Surgery

Radiation is commonly recommended after lumpectomy to reduce the risk of cancer returning in the breast. Treatment may involve the whole breast, part of the breast or the tumor bed, depending on individual risk factors.

4. Radiation After Mastectomy

Post-mastectomy radiation therapy (PMRT) may be recommended for patients with lymph-node involvement, larger or locally advanced tumors, or other high-risk features. Treatment may include the chest wall and selected lymph-node areas.

5. Regional Lymph Node Radiation

When required, radiation may include the axillary, supraclavicular and internal mammary lymph nodes. Treatment is carefully planned to protect nearby organs such as the heart and lungs.

6. Modern Radiation Techniques

  • IMRT: Precisely shapes and adjusts radiation dose.
  • VMAT: Delivers customized radiation while the machine rotates around the patient.
  • IGRT: Uses imaging to improve treatment accuracy.
  • 3DCRT: Shapes radiation beams according to the treatment area.
  • DIBH: A breathing technique that can help reduce heart radiation, particularly for some left-sided breast cancers.
  • Hypofractionation: Delivers treatment in fewer sessions and is commonly used for many breast cancer patients.

7. What Does Treatment Involve?

Treatment generally includes:

Consultation → CT Simulation → Treatment Planning → Radiation Treatment → Regular Review

The treatment is carefully planned to target the cancer while minimizing radiation exposure to nearby healthy organs. Each radiation session is generally painless and takes only a few minutes to deliver.

8. How Many Radiation Sessions Are Needed?

The number of sessions varies depending on the treatment plan. Many patients can receive shorter hypofractionated schedules, while some may require longer treatment courses. Your radiation oncologist will recommend the most appropriate schedule for you.

9. Common Side Effects

Possible short-term side effects include:

  • Skin redness or darkening
  • Dryness or irritation
  • Breast tenderness
  • Mild swelling
  • Fatigue

Side effects vary from person to person.

10. How Are Side Effects Managed?

Your radiation oncology team will monitor you throughout treatment and advise you on skin care, hydration, nutrition, activity and medications when required.
